Depression, Anxiety and Other Mental Health Conditions

Managing depression, anxiety and other psychiatric conditions within a primary care setting using an Integrated Behavioral Health (IBH) approach shifts the focus from siloed care to a “whole-person” model. This strategy addresses the reality that most mental health concerns are first identified in primary care.

1. We offer Collaborative Care (CoCM)

The most evidence-based integration model is the Collaborative Care Model, which relies on a team-based approach:

  • Primary Care Provider (PCP): The “quarterback” who manages the overall medical plan.
  • Behavioral Health Care Manager (BHCM): Usually a social worker or counselor who provides brief interventions (like CBT or PST) and monitors progress.
  • Psychiatric Consultant: Provides caseload reviews and medication adjustments for the PCP, often without seeing the patient directly.

2. Key Management Strategies

Integration transforms how depression and anxiety are handled through four main pillars:

Universal Screening & Identification

Instead of waiting for a crisis, clinics use standardized tools at every visit:

  • PHQ-9: For depression severity.
  • GAD-7: For anxiety monitoring.
  • SBIRT: Screening, Brief Intervention, and Referral to Treatment for co-occurring substance use.
  • “Warm Handoffs”

When a PCP identifies a mental health concern, they perform a warm handoff—physically introducing the patient to the behavioral health provider in the exam room. This reduces the stigma and “no-show” rates associated with external referrals.

Measurement-Based Care (MBC)

Treatments are adjusted based on objective data. If a patient’s PHQ-9 score hasn’t dropped by 50% within 8–12 weeks, the team proactively changes the treatment plan (dose escalation, switching medications, or intensifying therapy).

Brief Evidence-Based Interventions

Primary care integration emphasizes short-term, solution-focused therapies rather than long-term psychoanalysis:

3. Benefits of the Integrated Approach

Feature

Traditional Referral Model

Integrated Model

Access

Long wait times for specialists

Same-day access in the clinic

Communication

Occasional faxed reports

Daily team huddles/shared EHR

Stigma

High (going to a “mental clinic”)

Low (seeing a provider at the doctor)

Outcomes

Fragmented; high drop-out rate

Doubled rates of clinical improvement

4. Medication Management

In an integrated setting, the PCP initiates pharmacotherapy, but with the support of the psychiatric consultant. This allows for:

  • Standardized Protocols: Using first-line SSRIs/SNRIs efficiently.
  • Proactive Side-Effect Management: The Care Manager checks in weekly to ensure the patient isn’t stopping medication due to minor side effects.

The goal of integration

Key Insight: The goal of integration is not just to “treat a diagnosis” but to improve functional outcomes—getting the patient back to work, improving sleep, and managing chronic physical conditions (like diabetes) that are often worsened by depression.
